OPEN REGISTRATION for 2009-10 Season begins at 5:00PM Monday, July 20th
NOTE: Due to high volume expected on July 20th, in person registration is highly recommended

1. Find the classes that you wish to take on the schedule.
2. Fill out and sign the registration form.
3. Fill out and sign the Emergency Medical form (see combined form).
4. Initial and sign the policy acknowledgement form.
5. Calculate your monthly tuition on the pricing chart.
6. Calculate registration fee ($20 for one student; $30 per family for more than one student in same family).
7. Write a check for the first month's tuition plus the registration fee.
8. a. Register in person - bring the three forms and check to the academy.
b. Register by mail - mail the three forms and check to the academy address below.
c. Register by e-mail - send an e-mail to CDA sales (click button at right) with the student's name, class(es) taking, phone
    number and credit card information (see below). This will hold a spot in the class but signatures will still be required to
    complete registration.
d. Register by phone - call the academy (789 - 0052) with the name of the student, class(es) taking and credit card
    information. This will hold a spot in the class but signatures will still be required to complete registration.

Some general notes on registration regardless of the category your dancer fits into:
  • CDA accepts cash, checks, VISA and MasterCard.
  • CDA season registration includes one tuition payment and a registration fee.
  • Registration is not considered complete until this payment is made.
  • Despite our new facility with larger studios, classes are continuing to fill up fast. CDA will enforce a maximum class size in order to provide the best possible teaching environment. Class size is determined by age level, art form, class level and studio size. Register now to ensure your spot in class.
